Services Description POSITION SUMMARY: Person in this position will
be responsible for providing case management services including
assessment of service needs, care, service planning, linkage and
advocacy for clients enrolled in transitional living services.
Essential Job Functions include those listed below. ESSENTIAL JOB
FUNCTIONS: Case Management
- Deliver psychoeducational group services, case management
services, education and community outreach services, and referrals
to appropriate services and community resources as necessary.
- Assessment and periodic assessment of individual needs for case
management services.
- Assist clients in transitions between levels of care.
- Development and periodic revision of case management plan that
includes service activities.
- Communication, coordination, referral, linkage, and related
activities.
- Monitoring service delivery to ensure client access to
necessary services.
- Patient advocacy, linkages to physical and mental health care,
transportation, and retention in primary care.
- Maintain client files according to company policy and contract
requirements.
- Develop and maintain working knowledge of available community
resources and act as liaison.
- Maintain relationships with referring and supporting agencies
and represent the agency at various community meetings as
requested.
- Participation in facilitating ancillary services, i.e.,
continuing care, networking with community agencies, participating
in community events and developing and updating referral and
resource information, etc.
Community Relations:
- Establish and nurture positive relationships with community
members, leaders, and organizations.
- Act as a liaison between the organization and the community,
facilitating open communication and addressing concerns.
- Organize community events, workshops, and outreach programs to
promote the organization's services and foster community
engagement.
- Identify opportunities for collaboration with local businesses,
government agencies, and nonprofit organizations to enhance
community development initiatives.
- Must Embrace and embody the mission, vision, guiding
principles, clinical vision, and goals of WestCare Foundation;
and
- Perform all other duties as assigned.
Qualifications ESSENTIAL QUALIFICATIONS: To perform this job
successfully, an individual must be able to perform the duties and
responsibilities satisfactorily. The requirements listed below are
representative of the knowledge, skill, and/or ability required.
Experience and Competencies:
- Basic computer skills.
- Good documentation skills.
- Be able to multi-task.
- Excellent communication skills (written and oral).
- Must be culturally/linguistically sensitive to populations
served.
- Knowledge of substance abuse and treatment is desired.
- Ability to exercise good judgment and discretion.
- Ability to work well in a team environment.
- Adherence to the highest standard of ethical conduct,
especially to standards governing confidentiality.
- Professional appearance and demeanor.
- Must be culturally/linguistically sensitive to populations
served.
- Valid driver's license and ability to obtain and maintain
WestCare's vehicle insurance is required.
- Must be able to successfully obtain and maintain a criminal
background check.
- During your tenure with WestCare there may be new requirements,
including, but not limited to vaccinations that are issued by
local, State, Federal, and/or Funders that WestCare may have to
comply with. Should this occur Human Resources or appropriate
personnel will inform you.
- CPR Certification, First Aid Certification, and an annual
Tuberculosis test.
EDUCATION and/or EXPERIENCE:
- Two (2) years case management experience preferred.
- Basic computer skills.
- Good documentation skills.
- Be able to multi-task; and
- Excellent communication skills (written and oral).
- Graduation from an accredited four-year college with a degree
in Psychology, Social Work, Rehabilitation Counseling, or a related
field is preferred.
- High School Diploma required.
LICENSURE/CERTIFICATION:
- Not applicable to this position.
WORKING CONDITIONS:
- Work is primarily performed in an office or in a
residential/outpatient treatment setting.
- Some outdoor activities may be required.
- UA testing of clients is required.
- Must be able to de-escalate situations in the program.
