Job Summary and Qualifications

As a Medical Social Worker, you will assist in meeting the psychosocial needs of hospital patients by providing counseling, crisis intervention, and complex discharge planning services to patients and families.

  • You will respond to all interdisciplinary team and patient/family referrals within 24 hours during business days and Monday morning for weekend referrals
  • You will perform thorough assessments and develop appropriate plans of care for high risk patients with discharge planning or psychosocial needs
  • You will provide appropriate counseling to patients/families and refer longer term needs to appropriate outpatient services
  • You will respond to all crisis referrals from the Emergency Department or nursing units within 10 minutes by phone and within 20 minutes in person
  • You will interpret appropriate social and family information to healthcare staff

Experience

  • Previous experience working in a medium to large hospital setting – preferred
  • Previous crisis intervention experience – preferred

Education

  • Degree in Social Work at Master’s level

Licenses and Certifications

  • State licensure through the Texas State Board of Social Worker Examiners at a Master’s level
  • Accredited Case Manager (ACM) certification – preferred
  • Basic Life Support (BLS) certification as per St. David’s HealthCare policy – preferred
