Job Description:
Touchstone Communities is seeking a dedicated Licensed Social
Worker for our skilled nursing facility, focusing on providing
exceptional tailored care and support to our elders. The ideal
candidate will be instrumental in enhancing the quality of life and
overall wellbeing of our residents through personalized social
services. This role demands a compassionate approach and a strong
understanding of geriatric needs in a long-term care setting.
Responsibilities include assessing resident social needs, creating
care plans, coordinating with healthcare providers, and advocating
for resident rights. Conduct comprehensive assessments of
residents' social, emotional, and psychological needs. Develop and
implement personalized care plans in coordination with the medical
team. Provide counseling and support to residents and their
families. Advocate for residents' rights and ensure compliance with
all federal and state regulations. Coordinate with external
resources to assist residents with their social, emotional, and
health needs. Facilitate support group meetings and educational
sessions for residents and families. Strong interpersonal and
communication skills Experience in geriatric social work or with
elderly populations Ability to manage multiple cases effectively
Proficiency in documenting and maintaining accurate records Crisis
intervention skills 401k Must be a Licensed Social Worker in the
state of Texas Certification in Gerontological Social Work
